Every integration ends before the work truly does. This log exists for that gap — the stretch after the team disbands but before the last loose ends are finished. It's the answer to a simple question: what happens to any remaining tasks when everyone goes back to their day jobs? The End State Log exists to manage this final transition. It captures the work that continues after integration teams disbands so work is not orphaned and left uncompleted.

The log tracks every remaining action in one place. Each item includes an owner, priority, due date, status, dependencies, and supporting notes. The tasks spans multiple functions, including documentation, systems, finance, legal, HR, facilities, governance, communications, and culture. 

The log provides accountability when accountability is most likely to be lost. As the Integration Management Office winds down and team members return to their permanent roles, every remaining responsibility is formally handed to a permanent owner with a defined completion date.

The End State Log is the integration's final control document. It ensures the last commitments are fulfilled, institutional knowledge is preserved, governance transitions cleanly to the business, and the value created during the integration is fully protected after the project team has disbanded.
